New Patient Registration
If you have recently moved into the area covered by either of the Cornerways surgeries and you would like to register for NHS services please call into one of our premises and speak to our Reception Staff who will be pleased to help you.
What you will need to bring
It would be helpful to bring your Medical Card and be prepared to provide full details of your home address. We will provide you with a simple application form that you will need to complete and submit to enable us to make a provisional registration on our Clinical System.
If you are aged 18+ you must produce a utilities bill AND passport or driving licence (photo) as proof of residence.
If a family is registering, only the parents need supply these details.
We cannot register patients without these details.
Choice of GP
All new patients will be registered with the Cornerways Medical Centre and you will be entitled to express a preference for the General Practitioner of your choice. We have both Male and Female Practitioners. We will endeavour to facilitate your consultation with the Practitioner of your choice. However, there will be times when we are not able to accommodate your request. In these circumstances we hope that you will be prepared to meet with one or another of our fully qualified and experienced Clinical Team, all of whom will have access to your Medical Record and be able to discuss and resolve issues raised during your consultation with them.
What happens next?
We will forward important information to our local Primary Care Organisation that will ensure that your Medical Records are transferred from your previous GP to us.
It can take between four and eight weeks for your Medical Records to be transferred.
